Before the big day: Setting up your support network because human milk-feeding is hard

So you’re an excited (or nervous, or scared, etc etc etc…) individual expecting a new little one into your life. I’m going to call this ageless little one Buttons (I know they’re not a cat, but work with me). You and/or your partner would like to chest/breast/human milk-feed/re-lactate/lactate. However you are planning to do it, you want to provide Buttons with human milk, because, you know, everyone says it’s the best. Okay. So your devotion to providing the optimal nutrition is not the issue here. And actually, it’s not the issue with lots and lots of parents. We’ve all heard it before, we know that “breast is best”. But what happens when you’ve tried for 3 months and suddenly you just can’t cope? What happens when you’re sick and your partner is off on a business trip and you have two other kids at your heels? What happens if you’re transitioning or going through menopause? What happens if your doctor told you that people from your ‘background’ have less nutritional milk (which is so wrong, by the way)? What happens when you have nipple pain and mastitis and sleeplessness and anxiety and you just don’t have any answers and you feel alone in your journey with a helpless infant?

THERE IS A WAY! This is not to scare you off. You are making the right decision, and the experts aren’t wrong, human milk straight from the source is best (you are the source, just fyi). But what all those experts have forgotten is that it is also hard. We know human milk-feeding (HMF) is best, but the support to do it is missing when it gets hard and that’s where the medical community and governments and PSAs are falling flat. Some people find it easy right from the start and don’t have much of a problem, but generally, people are fumbling their way through this pretty independently, with issue after issue, making it up as they go along. We don’t want to ask questions in case we seem incompetent or hear something we don’t want to hear. We don’t want to ask for help in case we are putting someone out, but we don’t want to make mistakes because oh my god we can’t fail. We can’t do it alone, we have to ask questions. But you can do this, and it’s worth doing it, you just might need some help along the way.

And so the key is, after committing to producing and providing human milk for your babe-a-licious baby-kins or toddling tornado… commit to getting a support network.

If you can, do this before your new one comes. If you are planning to re-lactate (as in, inducing lactation without a pregnancy), you may already be researching how to do this (if not, I’ll have a post coming soon with resources for you!) and that often comes along with at least a small amount of medical community support. But we need more than that. Most new parents or caregivers are not surrounded by a hands-on community without asking for it. HMF a new one is hard. But it’s also worth it. And you can succeed, but your chances of success are increased exponentially with every addition of support you have around you.

Okay, so where can you get your support?

If you have a partner, start there. Talk about your feeding decisions. Make sure they understand how you would like to feed Buttons and make sure they’re on board. (If this conversation isn’t going that well, I’ll also have a post about ways you can navigate this… establishing and maintaining that feeding relationship is definitely one of the most important things you can do for Buttons.) Okay, check.

Friends and family. Historically, parents and grandparents are a wealth of information, but in the past couple of generations, knowledge around lactation and human milk went the way of the VCR and the roll-down car window… If you’re one of the lucky ones with milk-experienced family, use them! But be wary, because even the most supportive people can have misled information and ideas about milk, things like scheduled feedings, and when to introduce solids. “Well, it worked for my kids,” is a very common thing to hear, and unfortunately it’s just not always accurate, even if well-meaning. Do your own research, and again, make sure they know what your plans are for feeding Buttons. I have a friend who is expecting a second child and had a tough time emotionally with their first. About 8 weeks in, I was tagged into a group chat asking for everyone to rally around their family in whatever way we could. It’s not an easy thing for everyone to do; I know I’ve never been good at asking for help when I need it, but if you set up your net before you fall, it may be a much softer landing.

Find a local (or online) La Leche League International (LLLI) chapter. Most chapters operate in-person meetings on a monthly basis and anyone can attend (and you can usually bring your older kids if you can’t find childcare). Larger cities often have multiple chapters, and they are in over 70 countries worldwide, and if there isn’t one close enough to you, you can check out their online community here, or their Facebook community, both of which can provide buckets of support and wisdom.

Get online support! Nowadays we tend to depend on online communities as much or more than our in-person alternatives. Google is a great resource if you know what you’re looking for. LLLI is great and open to anyone, but if you are looking for support more specific to your own group or identity, head to my resources page. If you don’t see anything that jives with you, tell me and I’ll do the digging. Unfortunately for all the wealth of information on the internet, support for minority and marginalized groups still tends to be small and hard to find. The good(ish) news is, if you keep asking for it, it will eventually exist. And if it exists, I will post it. 

Alright, so you’ve got partner, family and friends onboard, plus a whole bunch of strangers in your city and around the world. What else is there? Your doctor, your OB or midwife and your pediatrician. SMH. This can be great or really tough. The truth is that most doctors have very little training with human milk-feeding, let alone with anything off the terribly beaten path in that world. Even OBs and midwives get very little. If you have a good open relationship with your doctor, fabulous. LOTS OF DOCTORS ROCK MY WORLD! But lots of other doctors were trained during the era of super-pro-formula and, especially in the US, formula companies still have their hands in the pockets of the medical industry, as much as WHO and UNICEF try to yoink them out. So it can be a battle to get your GP or medical professional onboard. But try. Have the conversation in advance. Tell them, much like your convo with your partner, that you are planning to do this, in whatever way you are planning to do it, and you’d like their support, and please don’t immediately tell you to use formula at the first sign of a problem. Even if your med-profesh isn’t totally seeing eye-to-eye, at least you’ll have laid the groundwork. My personal advice, if at all possible, is to find a doctor who is ready and thrilled to work with you to make your choices a reality. But I get it, for some people this may be impossible, out of the sphere of reality, just not an option, or simply more legwork than you are realistically prepped for. So, as a bare minimum, have the talk; at least they’ll know where you stand.

Anyone else? If you have the funding, or the benefits available, set up a meeting with a Lactation Consultant. International Board Certified Lactation Consultants (IBCLCs) are sometimes covered by health benefits plans. You may also be able to find doctors who specialize in HMF services, and a Lactation or Breastfeeding Specialist who is independently educated will likely cost less than an IBCLC. Ultimately, there are several programs that give people designations, but IBCLCs are the most regulated and internationally renowned. (I also have a post on dismantling the confusion of lactation professionals coming soon.)

Okay! Now you have your support network toolbelt on and ready at a moment’s notice. There will still be battles ahead, but maybe you feel a little better knowing that you have hands to hold and people to fall back on if you need them. You can do this. We will all help you.

Blackberry Raspberry Breakfast Muffins

I’m always looking for new ways to “dress-up” my oatmeal.  I can not just eat plain oatmeal unless I have a bunch of “stuff” in it.  I also wanted to make something that would be easy & healthy for Sean to take on his way out the door in the morning.  With that being said I came up with this vegan friendly oatmeal muffin.

1 1/4 cups whole wheat flour

1 1/4 cups steel cut/rolled oats (whichever you prefer)

1 tsp baking powder

1/2 tsp baking soda

1/4 tsp salt

1/2 tsp cinnamon

1 cup unsweetened applesauce

1/2 cup almond milk

1/2 cup pure honey

1 whole mashed banana

1 cup blackberries/raspberries (you can just use one type of berry)

Preheat oven to 375F

Combine all ingredients in large mixing bowl and pour into muffin tin.  Bake for 16-18 minutes.  Makes about 12 muffins.

Now if you are like me and love sweets in the morning you can add a chocolate topping to these muffins like I did.

1/2 cup vegan chocolate chips (you can use standard chocolate chips here too)

1/4 cup almond milk

1/2 tsp vanilla

Put all ingredients in a microwave safe bowl and microwave ingredients for 30 second intervals until its creamy.  Dip tops of muffins in chocolate or drizzle on top if you would like.

There you have it!  A really yummy, super easy oatmeal breakfast!

Lactation Cookies...Vegan Style

Vegan Lactation Cookies

It’s the new year and with the new year comes resolutions.  Well our family resolution was to eat and live healthier than we have been.  With that we decided to adopt a vegan lifestyle.  (I’ll blog about that in another post.)  I started to convert some of our favorite recipes so that they would be vegan friendly.  Most of them being baked goods.  Just because we want to eat healthy doesn’t mean that we can’t satisfy our sweet tooth!  And while satisfying my sweet tooth I might as well increase milk production too.  Here is my converted recipe for my favorite lactation cookies. 

Ingredients:

  • 4 tablespoons of Brewers Yeast
  • 2 cups of Whole Wheat flour
  • 1 teaspoon of baking soda
  • 1 teaspoon of cinnamon
  • 1 teaspoon of salt
  • 3 cups of steel cut/ old fashioned oatmeal
  • 1 cup of  chocolate chips (I found vegan chocolate at Trader Joe’s)
  • 3/4 cups Pure maple syrup
  • 1 cup unsweetened applesauce
  • 1 whole mashed banana
  • 2 tablespoons of Flaxseed Meal
  • 4 tablespoons of water
  • 1 teaspoon of vanilla extract

Preheat the oven to 350F.

1) Sift together dry ingredients, except oats and chips.

2) Mix together 2 tablespoons of flaxseed meal and water, set aside for 3-5 minutes.

3) Cream applesauce and maple syrup. Add bananas, mix well.

4) Stir in flaxseed mixture and vanilla to the syrup mixture. Beat until blended.

5) Add dry ingredients to syrup mixture. Stir in oats then chips.

6) Scoop or drop onto baking sheet. The dough is a bit sticky so use a spoon.

Bake for 8-12 minutes, depending on size of cookies. Makes approximately 6 dozen cookies.

Because this recipe does make a lot of cookies I usually flash freeze some dough balls in the freezer and then I bag them and have them available whenever I want.  Just form dough into balls and set in a pan in your freezer for about an hour.  After the hour (or longer in my case because I completely forgot about them) transfer them to freezer bags.

There you have my very first converted recipe!  Just because these are lactation cookies does not mean that only breastfeeding mommas can eat them.  They are really yummy and for the most part guilt free!

What is Dyspareunia?

Dyspareunia (difficult mating) is defined as genital pain that occurs before, during, or after intercourse.

This is not a well understood condition, and it can be lifelong or acquired.

If dyspareunia has not always been present, it usually occurs ten years after the start of sexual activity.

How Common is Dyspareunia?

“Dyspareunia is one of the most common sexual dysfunctions seen by gynecologists and is estimated to affect about two-thirds of women during their lifetime, (Marinoff, S.C. and Turner, M.L.C., 1993).”

The medical literature does not quantify the number of men faced with this problem.

Dyspareunia Sufferers should Consider the Mind and Body

Dyspareunia can have psychological as well as physical factors.  It is important to consider both the mind and the body in any treatment regimen.

According to Novak’s Textbook of Gynecology, 12th Edition, by Jonathan S. Berek, 1996:

 “Psychological factors contributing to dyspareunia include:

  • Development factors, e.g., an upbringing that invested sex with guilt and shame;

  • “Traumatic factors, e.g., childhood sexual abuse or other sexual assault;

  • “Relationship factors, e.g., anger and resentment toward a sexual partner.”

“The repeated experience of pain during intercourse can cause marked distress, anxiety, and interpersonal difficulties leading to anticipation of a negative sexual experience and eventually to sexual avoidance (American Psychiatric Association, 4th ed. 1994).”

Vaginal Infection \ Prostate Infection

“Vaginal infection is the most common cause of successfully treated dyspareunia,  (Glatt, A.E. 1990).”

In men, infection of the prostate is the most common cause of successfully treated dyspareunia.

Other common physical causes are genital glands and adhesive bands that swell only in relation to intercourse.

Dyspareunia|Hypersensitivity or Irritation of the Clitoris or Prostate

Stimulation of the genitalia by sitting, and conditions such as hypersensitivity or irritation of the clitoris or prostate can be causes of frustrating pain.

Other anatomical causes of dyspareunia in women can be a rigid hymenal ring or scar tissue in an episiotomy scar.

Irritation from Over-the-Counter Products

Dyspareunia can also be caused by irritation from over-the-counter feminine spray products, douches, and contraceptive devices (i.e., condoms, sponges, diaphragms).

Dyspareunia can be Minimized with Water Soluble Lubricants

A common cause of dyspareunia is inadequate sexual arousal.

This condition may be resolved with adequate lubrication, increased foreplay, and counseling, if necessary.

When using lubricant, always use water-soluble products, such as KY Jelly or Astroglide.

Dyspareunia can be Associated with Menopause and Lactation

Finally, pain associated with intercourse can be the result of vaginal atrophy from estrogen deficiency in menopausal or lactating women.

This condition can be diagnosed by your physician and can be treated with vaginal or systemic estrogen replacement therapy.

Milk

A few weeks ago I received a random text from Missy.  Missy has been my bestie since we were in Girl Scouts at the age of 8.
“Matthew is trying to tell me that we are NOT the only species that drinks a different species’ milk. Tell me he’s wrong”  the text read.

“Matthew is definitely wrong” was my response.  Knowing Missy as I do, I imagined she was enjoying her victory at that moment.  Matthew, her (usually) very knowledgeable husband, had to accept his defeat with this one.

When I first started to learn more about breastfeeding, I remember hearing someone say this exact thing. We are the only species that drinks a different species of milk. At least intentionally.  I really had to give this consideration at first – think about other mammals, whether or not I had ever heard about them feeding from other species. When you really look at it that way, it seems kind of strange to think that we are the only species that intentionally and willingly drinks the milk of another mammal, and consider it normal. Yet, feeding the milk that our body creates especially for the baby that we birthed is strange to some.  Am I the only one who sees this as odd?

I have no idea what sparked the discussion between Matthew and Missy about this very topic. Honestly, I have known Missy for so long that I didn’t even ask why they were arguing about human milk when their youngest child is a teenager and had weaned long ago.  This week on the Badass Breastfeeding Podcast, Abby and Dianne talk more about milk because this is a question we get quite often. Not just about human milk, but about when to start babies on another species of milk and how to go about making that switch. Or do you even have to? Want to hear more about that? Tune in this week!

Oatmeal Scotchies

While in college, we all would love opening our mailbox to find the package slip. No, it didn’t mean my Amazon order arrived (that wasn’t really a thing back then); it was a care package. I remember getting my first package within a few weeks of my arrival at Hobart & William Smith from my Grandma Bunny. Knowing my Grandma, I was sure it was some baked goods of some kind, and I ran back to my dorm to see what it was.

Even before opening, you couldn’t miss the smell of popcorn. I opened the box to find my grandma used actual popcorn as packing peanuts, and hidden within the popcorn was a bundt cake, cookies and even a sheet set (all thankfully in plastic). My grandma would repeatedly send me these popcorn boxes while I was in school, and the smell of popcorn would always draw my friends over for a visit and a treat. All, including the popcorn, was always devoured by me and my friends.

One of my favorites cookies of hers were her Oatmeal Scotchies (aka oatmeal butterscotch cookies).  And don’t worry –  if you’re not a fan of butterscotch, you  can easily substitute with chocolate. Unlike my Chocolate Chip Cookie recipe, I make these bite-sized, so they’re much more manageable for a snack. I used to always make a batch of these after my son’s birth since they were a quick, easily accessible snack. Unlike my grandma’s recipe, I added in flaxseeds and brewers yeast to help with self care in those early days, but both are totally optional when making!

1 cup [2 sticks] butter
1 ½ cup packed brown sugar
1 ½ cup all-purpose flour
¼ teaspoon salt
½ teaspoon baking powder
½ teaspoon baking soda
5 tablespoon brewers yeast (optional)
3 tablespoon ground flaxseed (optional)
1 ½ teaspoon vanilla extract
1 egg + 1 egg yolk
11 oz butterscotch chips

Preheat the oven to 350 degrees. Line two baking sheets with parchment paper.

In a medium bowl, whisk flour, salt, baking powder, baking soda, brewers yeast and flaxseed together, set aside. In a standing mixer, cream together butter and brown sugar until fluffy. Add egg and egg yolk in and mix well. Add flour mixture and mix until batter is just combined. Gently fold in the butterscotch chips until the butterscotch is just incorporated. Use a small spoon to form 1” balls.

Bake in the preheated oven for 12-15 minutes until golden brown, taking care to not overbake. Remove baking pans from oven and let cookies cool on a wire rack until cool.

Note – to make ahead combine and store in freezer for up to 3 months in a airtight bag. Bake when ready to enjoy!
